"Due to an administrative error, the waiting list data previously provided to the Regional Health Forum West Members was incorrect. This occurred due to the miscalculation of the OT excel spreadsheet whereby only one tab of the excel sheet was provided rather than the full total. This has since been identified and rectified and we wish to apologise for this error."
